Insurance and Rates

We accept the following Insurance Plans:

• Anthem 
• Medical Mutual of Ohio
• UMR
• UHC

We do not participate with Medicare however, our Medicare patients will only be charged Medicare's allowable rate and we will provide you with all of the paperwork you need to file a refund request.  

We are not participating with any Medicaid plans.  These patients will be treated as cash patients.

We are not participating with any Anthem Tri-Health plans.

Our cash rates are available upon request.
